When you call Urology Associates to schedule an appointment. We’ll work with you to schedule an appointment with the physician of your choice. If that physician is not available, we’ll be happy to suggest another qualified partner of the practice for you to consider.

When you arrive at the office, please bring the following information:

• New Patient Registration Form
• Patient Evaluation Form
• List of current medications
• Any medical records or x-rays related to your condition
• Insurance card(s)
• Insurance referral if required by your plan.

If you are being seen for a infertility issue, please also complete the Infertility Evaulation

Our office will call to confirm your appointment at least 2 days prior to your appointment. We ask that you please provide 24 hours notice if you are unable to keep you’re appointment.
